I understand that individual health insurance covering the dancer named above is a requirement to attend the program. I grant Talent On Parade, LLC and camp staff the right and authority to make emergency medical decisions in good faith, and in the best interest of my dancer for the program dates of July 21-28, 2025. I also agree to release and hereby indemnify Talent On Parade, LLC and their agents from any and all contracts, claims, suits, actions, liabilities and or injuries arising as a result of participation in this workshop, and/or as a result of medical/safety decisions made in the case of an emergency. I further release Talent On Parade, LLC and their agents from responsibility for any and all loss or damage, court costs and/or legal fees. By my electronic signature below I also affirm that I have read and understood all appropriate documents, rules and regulations as regards to this workshop. I also understand that payments for the program are expected as follows: 1) A $250 non-refundable deposit is due immediately following receipt of the PAYPAL invoice generated by this application; 2) Payments must be made so that invoice balance is $810 or less as of May 9, 2025; 3) Additional payments must then be made so that invoice balance is $300 or less as of May 30, 2025, and all payments (not just deposit) become non-refundable as of that date; 4) Additional payments must then be made so that invoice balance is $150 or less as of June 13, 2025; 5) Final payment deadline is on or before June 27, 2025; 6) All payments must be made via the PAYPAL invoices emailed from TOP. At any time you may share a PAYPAL email invoice link with others who wish to “donate” towards your dancer’s tuition. Failure to meet any payment deadline indicates express wish not to participate, and forfeiture of any and all payments made up to that point. I also understand that refunds will not be provided due to program cancellation/postponement as a result of any national emergency, acts of God, war, acts of terrorism, medical emergency, weather-related issues, force majeure, or other circumstance(s) in which TOP has no control. In lieu of refunds, credit in the amount of payments received will be held on a studio account instead. By my electronic signature below I also grant Talent On Parade, LLC and camp staff the right and authority to transport my dancer as necessary for any and all activities related to the 2025 TOP Go Program workshop during the dates of July 21-28, 2025. Finally, I understand my dancer’s participation can be terminated immediately at the sole discretion of any member of the TOP Go Pro Staff for violations related to, but not limited to, the following: consumption of alcohol, illicit drugs, smoking, vaping, or promiscuous activities. Upon such termination I agree to retrieve my dancer immediately, at my own expense, and without refund.
