2018 PROgressions Registration Form

  • I understand that individual health insurance covering the dancer named above is a requirement to attend the program. I also agree to release and hereby indemnify Talent On Parade, LLC and their agents from any and all contracts, claims, suits, actions, liabilities and or injuries arising as a result of participation in this workshop. I further release Talent On Parade, LLC and their agents from responsibility for any and all loss or damage, court costs and/or legal fees. By my electronic signature below I also affirm that I have read and understood all appropriate documents, rules and regulations as regards to this workshop. Finally, I understand that payments for the program are expected as follows: 1) A $250 non-refundable deposit is due at the time of application; 2) Payments towards the program must total a minimum of $400 as of June 5 and become non-refundable as of that date; 3) Any remaining tuition balance must be paid-in-full on or before July 3. Deposit payment must be made online via the PROgressions Deposit Payment Form. A PayPal invoice for additional payments will be emailed after deposit is received. If at any time a payment is mailed (not recommended) it must be trackable and received in the TOP office per the deadlines above. Failure to meet any payment deadline indicates my express wish not to participate, and forfeiture of any and all payments made up to that point on my behalf.
